This is from an awning window to a breezeway. We could not find and name or numbers.

Thanks for posting. Could you show us a picture of the operator from another angle that allows us to see the installation hole pattern? This is a key feature for determining a replacement. Many thanks.

Holes measure 5 3/16" on center lengthwise, 1 1/8" on center across. Thank you!

I'm afraid that I couldn't find anything in this style that matches your dimensions. The closest I could find was our 39-025. Could you possibly use this as a replacement? Look it over and see what you think.
