Number on back of gear cover is #45002

Extends out approximately 18"

Bracket that's attached to window is 12" long

Thanks for the photos! Your operator appears to be the 39-083 21-1/2" Dual Arm Crank. Take a look over the dimensions and design to confirm, but I am confident this is the replacement you'll need. The compatible bracket is part 39-170 and the compatible crank handle is part 39-114L.
