I suspect you might have the 39-025 or 39-061, but I've listed a bunch of possible candidates on this page. Ultimately it will come down to not just the hole pattern on those tabs, but the style of the link arm and the hole at the end of it as well.

The 39-194 has that slight offset just like yours. Just review all the dimensions listed to confirm it matches up. Note that the overall length of the link arm isn't too important; some play is allowed for with these awning operators.
