I’ve measured the length at 20 and 1/4 inches x 1/2 inch wide. And weighed the sash at 12.75 to 13 lbs…. This seems to line up with the stamp of "1915" , so I selected the closest weight option... S745-20. Let me know what you think would be best. Also, I’m not sure how the cord gets attached to the frame, so if you can point me in the right direction, that would be great.

Yes, we agree with the S745-20 with spring stamped number (19.25-4) for a sash weight between 13-17 lbs. With needle nose pliers you would pull out the 16-027 Peachtree nylon plug attached to the cord from the window track. Then you would unscrew the top and bottom screw holding the balance in the sash. Install the new balances on both sides of the sash, then with self locking pliers pull the cord out a few inches to give you room the attach the 16-027 plug into the hole in the track. Carefully remove the locking pliers from the cord so it doesn't snap back at you. Repeat other side.
