Balance assembly and pivot shoes for Simonton window

Hi i am looking for parts

Balance assembly, pivot & shoe double hung Simonton 550

Replaced all windows in 2009. Last year when opening windows. Two of them parts flew out and either the top or bottom slams down.

This year it happened to a few more windows.

Attaching pics

In need of the entire balance shoe pivot & what ever else goes with to install & operate my windows. Also that pivot has a # 07 37
