This looks like it might be the Series 377. You'll need to show us the bottom attachment, as well as tell us the length of the metal channel. Any stamp code on the metal channel would also be useful to know. From there, we'll do our best to narrow down what you need.

Thanks for getting back to us. While similar in appearance to what Bob suggested, this is actually a custom 18" balance with a 19-041 Top Fitting and a 15-005 Bottom fitting, along with a stamp option of 1710, which is for a window sash in the weight range of 5 through 8 lbs.
