Balance slide selection

Question

Hi, I need help to select new balance slides for my 12 year old windows. The only label I have shows the windows as series 3500 / 3550 HR 30 36x64. Photos have been added and I hope you can guide me in the right direction. The measurements for the slides are 1 1/4 X 2 X 1/2
