Looking over your photos and information, our S795-28 channel balance may be the balance you have been looking for. This balance has a screw on top clip. Look it over and see if this looks like a winner.

As for the bottom balance shoe, depending on the size of your shoe will determine if the 15-194 or 15-195 t-lock balance shoe would make the better replacement.

Lastly for the spring option, you would go with 28D to accommodate your "28 DD" marking.
