To further review, are you able to show us a picture of the top nylon attachment? Can you also confirm the depth of the bottom shoe to see if it matches to the diagram below?
For the top sash guide, do you have an undamaged version still installed that we could view? It would be best to see a clearer image of the installation hole placement. Thank you!

Your balance uses the 19-024 top attachment and 15-095 bottom shoe. We can provide this configuration via a custom order. Please confirm that the length of the metal channel excluding the nylon attachments is 28 inches long. How many balances are you in need of and I can send over an order link with pricing for your review.
For the top sash guide, I am hesitant we can provide an exact match. Do you have a picture of an undamaged version installed on the window that we could see? Thank you!
