Thanks for your post! I am confident we have the balance you need but I will need to see the photo of the bottom attachment from the balance, the photos provided only show the balance top. I'll also need to see a photo of the stamp from the metal channel.
Your top sash guide looks like an exact match for the 18-180 1/2" Top Sash Guide. Review the dimensions to see if this is the right fit for your repairs.

Here are some more photos. I submitted more photo but they went to a different reference number. Anyway, thank you for your help. Besides the balance, I'm also looking for the fuzzy felt weather stripping on the window, and a small L -shape guide on bottom.

I'm sorry to say we would not be able to provide you with a replacement for the L-shaped bracket installed to the. Regarding the balances, it appears you may have a shoe with a 7/8" depth. If so, the replacement balances will need to be custom assemblies.
In addition to confirming the shoe depth it could also be a good idea to confirm that your sash weight falls between 23 to 28lbs.
The last piece of information I'll need to know is the exact number of balances you intend to purchase. There is a quantity minimum of (2) balances.
