This looks like either a Series 351 or Series 390 Channel Balance. The only difference between these two balances is the 'depth' of the bottom nylon shoe, so compare both with your old hardware to see what matches best.

A quick learner from Office building says:
Hello- Thank you for getting back to me. (4) S351-29 : 29" Series 351 Channel Balance Is what I was recommended. Would you agree? Thank you

Bob from SWISCO responded:

Her Cherie. If the metal channel, not including the nylon fittings, is 29" long, and if the bottom nylon fitting matches our 15-002 1" Deep shoe, then yes, the S351-29 is correct. You'll also have to choose a stamp option. Look for a stamp code on the metal channel that confers with one of the options listed. If there is none or you are unsure, obtain the exact weight of the window sash. You can then match that exact weight with one of the weight ranges listed alongside each stamp option on the product page.
