Bi-Folding Door Hardware

Hi, I'm looking for hardware to use with this door. The pivots are okay but the guide pins and the guide brackets are destroyed. Can you guy's help with this?

We will try to help. I'm having trouble locating this exact part, though. Could you show us the nylon cap that fits on the pins? Can you also tell me their measurements? Finally, do you know the name of the door manufacturer?
