Blum Style Hinge

Question

Along with the accompanying photos there are two stamped numbers in the hinge to be replaced: S8590 and 80059. No manufacturer name within the part. Just identifying this hinge would be a huge accomplishment. Should a photo of the hinge out of it's home be required, let me know.
