Bottom Pane connection to weight system

Can not find the manufacturer of this window and one of them has lost all the hardware on the bottom pane that attaches it to the weighting system that holds the pane "up". Both metal slides that connect the pane to the sliding system cracked, broke and disappeared. On the left side, the blue insert is missing as well (picture shows both left and right side weighting system connections and you see only one has the blue insert).

Can you help identify the manufacturer and potentially help with a replacement part?
