Broken Sash Cam

Question

The sash balance springs up past the window. Found the broken sash cam, looks like the plastic broke around where the screw would mount. Found a label on the window that reads Series: 437/438; H-LC35-54X96; H-LC50*-52X76 (Modif). I found other entries but they only provide the part number for the sash balance. I am looking for the top cap.If you could provide a part number for the replacement sash cam it would be greatly appreciated.
