Broken balances and top sash guides

Question

I was able to remove the sliders and the cover has totally disintegrated.  I?d like to know what kind of sliders and covers to buy?  Thanks!

Thanks for posting! Your top sash guides look like our 18-124P while your balance system resembles our S380-34. Look this hardware over and see what you think.
