Can you please help me find the correct replacement hinge.

Hello, thank you for posting,

I believe you have the 79-132 pivot set used on YKK commercial doors. Looking at your door I can see a label at the lower left side of the door bottom. Check to see if that label is "YKK". Take a look and compare the 79-132 pivot set along with the related videos on commercial door pivots on the website.
