Thanks for the photos! The operator you have here resembles the 39-032 EntryGard Dual Arm Operator, the opposite handed version is the 39-031. Take a look over the dimensions below to confirm that this is the correct replacement.

These operators are compatible with the 39-107 cover, used along with the 39-114L non-folding or 39-205 folding crank handles. Another replacement option is the 39-107FK Truth Folding Handle and Cover Kit.
