The door is a Reliabilt vinyl sliding patio door, model no. 3384881. It was purchased and installed in March 2013. The outer handle has cracked beneath the lower bolt. I obviously need a new door handle. Pictures "door handle 1" and "door handle 2" are of the outer handle; "door handle 5" is of the interior handle.

Additional information which I forgot to include: the base of the handle is 9 1/2 inches long, and the distance between the center lines of the 2 bolts is 4 inches.

Thanks for posting. This looks like our 82-086 to me. The hole spacing should be the same, and the key lock is in the same position. Please compare.

The 82-086 was a perfect match. My handyman installed the replacement for me. He was so impressed that when another client of his needed the door handle replaced, he asked where I had found the replacement. I gave him your website info and the model number of the handle.
