Broken plastic rear drawer slide mount

I am looking for a replacement rear mount slide bracket which has a broken edge.

I do not know the manufacturer. Can you provide a replacement?

It is 1-1/2 inches x 11/2 inches square x 5/8 deep and the slide pocket is 3/4" tall X 12/16 wide x 1/2 " deep

I don't see anything on - line that looks identical.
