Broken top sash guide

Question

I have a broken top sash guide, seems to match your 18-166. When I removed it from the window, only one screw was holding it in place. Screw was in the top hole only. Should it have 2 crews and if so, maybe that is what made it fail. Tx

It's not necessary to have both screws installed in the top sash guide. They are made that way so that they can be used in different applications. Failure is usually caused by age and fatigue. I don't recommend drilling another hole because you might break the glass.
