Window manufacturer is unknown. Numbers on sticker on window include DH-R25 /DH-R30 and MST: 4080/3850

Your sash lock reminds me of our 90-310 at first glance but you'll need to confirm that the dimensions on our product page match up to yours, paying close attention to the 2-/4" distance between the installation hole centers.
