I need several Caldwell Series VS-5A channel balance replacements, but cannot determine which option. MFGR RYL-017-1. MST 4-0 x 6-0. 1/2" wide. 5/8" deep. 34" long. Appears to be Series 350 because shoe 15-004, top 19-002, and top sash guide 18-110 match shape and dimensions of what I have. The Caldwell channel is stamped 33 4 HP. The non-tilting sliding window is 48" x 36". The sliding bottom portion of the window weighs 24.5 lbs. Please help me confirm the correct replacement balance.

Without seeing your balance attachments, and going with what you believe are the correct balance attachments, the S350-34 Channel Balance with stamp option 3340 appears to be the replacement you're looking for.
