The channel is stamped Caldwell 33 3, and the actual length of the metal portion is 34" Both ends have wings. The sash weight is 20.7 lbs. I suspect that I need the 380 series with a 3330 sash weight. Am I correct?
Do the parts come with the wings attached, or do I have to purchase them separately?

We'd be happy to help but we would need to see photos of your original balance uninstalled to narrow down your options since the stamp code only indicates the strength of the spring. If the balance is stamped with 86, that indicates that your balances would need to be custom-ordered.
To confirm, we would also need to see the connecting point between the spring and pulley inside the channel. We'll also need to know the number of balances you need, with the minimum being two. Thank you!
