Caradco operator cover and maybe crank handle

Question

I'm looking to replace one operator cover for a Caradco window. The photos show the cover off, and on. Dimensions are approximately 5 inches long by 1 inch high by 1 inch deep.

Also, on our crank handles, the knobs have come off all of them. We have the knobs but don't know how they are supposed to stay on.
