Casement Crank

Question

I am looking to replace this crank operator. it is 5" 14/16 inches end to end. the box part is 4.25 inches. It unit is unusual in that the screw brackets are oriented differently on each end.

We can replace this operator, but I cannot identify what you have from this photo. Can you remove the entire mechanism so we can see the link arm? That will be a very important clue.
