Your lock reminds me of our 38-208 but compare our dimensions with yours to confirm this matches what you need. As for the operator, I'm afraid your spline can't be replaced since it broke off so you'll need a new operator. We can help but we'll need to see a photo of your operator with the window open so that the arms that move the sash are fully visible.
