This looks to be an awning operator similar to our 39-083, 39-140 or 39-143. All these operators have a similar design but differ in their 'wingspan' dimension, so compare each to your old hardware to see what matches best. For a new handle, see the 39-114L. You also seem to show a keeper for a sash lock in your photos; compare it to the 38-277 and see if it matches up.
