Hello there. This looks like the 39-264 Andersen Split-Arm Operator. Please compare.
For a new handle, see the 39-286; for a new cover, see the 39-152. Note that if your window was made between 1995 and 1998, you must use the 39-362 Adapter Plate to install this.
We do not carry specific screws for this operator, since this depends as much on your window as it does the mechanism itself. A hardware store should be able to supply the proper fasteners for your application. Finally, we do not sell wood trim. I apologize for the inconvenience.
