It looks like the worm gear stripped and pulled out of your operator base. This isn't supposed to happen, unfortunately, and once it does it cannot be repaired. You will need an all new operator entirely.
We can help, but to begin I will need you to take the operator out of the window and show me a clear picture of the complete mechanism without obstruction. I will then do my best to match it up to something in our catalog. Let me know if that will be possible!
This is a huge help, thank you! I think you have something like our 39-083. Just note the "elbow to elbow" measurement, which is 21-1/2". If yours differs, let me know. We have other sizes so it's no big deal at all.
You can actually reuse your handle once you take it off the worm gear, but just in case you wanted a new one then I would recommend the 39-205.
