The operator shown in your photos resembles the Entrygard 39-031 and 39-032 at first glance, which use the 39-107FK handle and cover kit. The determining factor would be how the length of the shorter link arm compares to yours. You'll also notice that the link arm on our part has a downward offset, while yours appears to have an upward offset. Our operators can still work but keep in mind that you'll need to manipulate the arm to attach it to the sash bracket. Take a look and see what you think. If you need new hinges as well, take a look at the 36-048 set. Thank you!
