There is nothing wrong with the hardware! I can partially retighten the bushing and it works fine until the bushing gets loose. Surely one of your "experts" knows how the original bushing was installed and tightened.

That bushing is machine pressed at the factory. Once it strips out, like yours has, it can't be repaired or replaced. Not unless you have a manufacturing plant in your garage, anyway. Casey is 100% right: you need an all new operator.
