Need (4). 2 left, 2 right. Can't determine manufacturer. 5" wide, 2.5" deep. Notice hole on end of crank to receive bottom of window unit.

The uninstalled operator shown in your photos is similar to the Encore 39-220B, which is used along with the 39-375 Encore Truth Folding Handle with Cover. We offer the 39-221B and 39-376 opposite-handed versions of these parts as well. Look over the dimensions below compared to your assembly. Thanks for posting!
