Center Monorail Drawer Slide

Hi, I have a wooden center monorail guide drawer slide that needs to be replaced. The unit is: - 16 1/2" in length - width at the ears 1 1/8" - width at the neck is about 7/16" - height at each end 13/16" stepping up to 3/4" through the center portion (I could make either height work. - There is also a center groove at the top, between the ears. please let me know if you need any additional information.
