Hey Jim. Looks like you need a 15-192 Pivot Shoe, which uses our Series 170 and Series 171 Coil Balances. The video below explains more. 

The Series 170 and Series 171 both cover a different range of spring weights, you can view on each balance's page. For instance, the S170-335 is for 5 lbs of weight individually, whereas the S170-408 covers 8 lbs individually. You should be able to go by the stamps you listed, as well as the whether or not the springs are Series 170 or Series 171, to make these determinations. If you're uncertain, give us the exact weight of the window sash, and we can work out what you need.
