Certainteed double hung

In 1995 I installed Certaineed vinyl double hung replacement windows. Five years ago the left side glide shoe broke on the upper pane.We were able to get it up and lock it shut. No getting this thing cleaned! The bar code on window is 092400 020016. Window 39.75x45.25.i have been searching for a replacement part for a long time. Can you help me
