I have a wooden sash, double hung, non-tilting window with a bad channel balance. The part is stamped with "20 4 FS CALDWELL".
Size of the metal channel (not including plastic top/shoe) is 5/8" x 21". Weight of the sash is 18 pounds.

Hey, Jim. Are you sure you have the 15-003? This is not a common combination, and many people confuse the matter. I think it's more likely you have either our 15-001 or 15-002 shoe.
Now, as you can see, there is only one difference between these two shoes, and that is their depth. See the picture below for detailed dimensions. The Series 351 uses the 15-002 while the Series 390 uses the 15-001.

My balances are a different brand than Swisco. I used a caliper to measure, the shoe is definitely 1-3/16" long.
If I use the 15-001 it's only 1/16" smaller. Do you see any problem with that slight difference? If not I could order:
