Channel Balance 30 3 Caldwell FM

I need to replace the channel balances in a couple of windows as the pulleys inside have broken. The windows have the same channel balances. Here are the details:

Stamp on channel balance: 30 3 CALDWELL FM
Channel balance dimensions: length 31" (without guides); depth 1/2"; width 5/8".
Top guide: width 15/16" (tab to tab); top width 9/16" (above tabs); total length 1-1/4".
Bottom guide: width 1" (tab to tab); width 9/16" (above tabs); total length 1-1/8".

I've attached pictures of the top and bottom guides.
