Channel Balance for Jeld-Wen windows

Question

I need help getting the right replacement part please. They are Jeld-Wen windows. The original invoice describes them as 4050 single hung which I think refers to the size which is 47.5 w x 60h. I need the channel balances which, as you can see in photo, has the #26-5 ID5 CMC 45
