It looks like you have either our Series 380 or Series 385 channel balance.

The difference between the two balances lies in the bottom shoe attachment.

Comparison of 15-004 and 15-005

The series 385 balance has the 15-004 shoe which is slightly larger from front to back, as illustrated.

If you measure your shoe as it is pictured here, you should be able to determine which one you need.

Dan, after conferring with one of my part experts, we believe your shoe is consistent with our Series 380 channel balances and the reason your shoe looks different than what we show is your shoe has been worn down over time, causing the "heel" of your shoe to flatten out.
