Thanks for writing in. See our S380-30 or S385-30. These look similar to your balance; the only chief difference between these two possible replacements is the depth of the bottom nylon fitting, or what we call the shoe. The shoe depth can be thought of as the 'heel-to-toe' measurement. Compare both your old hardware to see what matches best. Based on your stamp code, you will need a stamp option of 2960. 

Your Top Sash Guide is hard to determine from these photos. Is there an undamaged version you can show us? Closer pictures of the front and side of the guide, like mugshot pictures essentially, would be most helpful. From there, we'll do our best to narrow down what you need.
