Based on your pictures, I think you may have our Series 350 channel balance. This comes with the 19-002 top and 15-004 bottom.

Considering your sash weight is 30 pounds, I recommend the 3350 option. You can see a chart on the S350-34 store page that displays how much weight each option can support.
