Channel balance 1/2 inch Alcon single pain single hung window. 37 inches long. Stamped 3606. I think it's 380 series.

It would depend on the depth of the shoe. It's hard to tell since yours is damaged. Do you have an undamaged one, perhaps in the other side of the window? If so, I recommend measuring that to be sure.
