Channel balance T- Lock Shoe

My tilting window bottom sash is getting stuck and would not rise above a foot from the bottom. The balance on the left was jamming and the culprit seems to be the plastic device which houses the metal shoe in the last picture. The white plastic insert designed to hold the metal shoe in place in it's housing also came loose and jammed the channel. Stamped on the metal channel, which is 34" long and 1/2" square, is 33B, BSI, DB. The plastic housing which holds the metal shoe is bent and, if replaceable, would probably correct the problem. Do you have channel replacements/individual parts for this window or can you refer me to a supplier? This house was built in 2010. Thank you for your help.

I had expected this search for a solution to my window issue to be lengthy and tedious. Instead, that was the most efficient solution I could have hoped for. The shoe seems to be an exact match and I have ordered 4 shoes. I also see that you provided installation instructions. It would be terrific if every customer service process with other companies worked as seamlessly as yours. The only other issue I can see that might have to be addressed is a slightly corroded spring. If that is an issue I will be back in touch.
