Channel balance
It appears one of the balances is jammed in the window track. That said, you'll likely need to replace the top sash guide. I'm happy to recommend a replacement for it, but I'll need to see an undamaged version to identify it. Please see the tutorials below for reference.
This balance is a match for either the S380-30 or the S385-30. To confirm the right series, you'll need to measure the depth of the bottom shoe.
Our 2940 stamp should work in place of yours. However, you may want to double check that the sash weighs between 23 to 28lbs.
Thanks for the additional photos, these are sufficient. Your top sash guide reminds me of the 18-166. To determine if this is the correct replacement carefully look over the dimensions and double check them.
