Channel balance replacement

I'm trying to identify these top and bottom guides. Do you have them available? They're on a vinyl non-tilt, 1/2" (9/16") wide, 5/8" deep, 16" long guide with the numbers "1530 BSI" stamped on it (10-12lb sash weight I think).
I need to replace two balance and top/bottom guide sets. I have more pictures available if you need them, but could only attach 5 here.

Thanks for posting. Before we can offer any suggestions, I will need a little bit more information about the bottom shoe. Please refer to the chart below and let us know which one is yours.

Diagram on how to choose between the 15-001, 15-002, 15-004, 15-005, and 15-029 channel balance shoes.

Thanks for the quick response. The bottom shoe seems to be # 15-004, based on wings and measurements.

Please take a look at our S385-16 with a 1530 stamp option.

Thanks Paul. That one looks close, except for the top guide. The top guide I have is square shaped, flat across the top. The one you suggested has a sloped top/front edge. Other than the sloped edge, it looks perfect though. Would that guide still work even though its not the same shape?
