Thanks for your question. This kind of balance is identified exclusively through the style of the nylon ends. It looks like you have the 19-005 top and I'm guessing the 15-004 bottom or shoe. The shoe could also be the 15-005. If you have the 15-004 shoe, we suggest our Series 385 balance. If you have the 15-005 shoe, then it's the Series 380. Check your measurements against ours and see which one will work.

Thanks for your reply to Julia. She is helping me out with this. By your description, I believe we do have the Series 380 Channel (27") with the 15-005 shoe and 19-005 top. Here's a picture from the back side of the channel with the numbers on there. Besides buying two shoes, two tops, and two channels, are there any other things I need to buy for this one window?

It really depends on what else is broken. These kind of windows have any number of parts that interact with the balance, but if they're in good condition there's no reason to replace them. I would give the sash a thorough once-over to see what else is damaged.
