Thanks for posting! Can you please tell me what the length of your metal channel is? Make sure not to incorporate the plastic attachments in your measurements.
Judging by your stamp, it may be 30". If you can confirm that, please take a look at our S385-30 channel balance with a 2930 stamp option and see what you think.
As for the top sash guide, please take a look at our 18-166. Does that match your hardware? If you're not sure, please let me know your guide's dimensions and I will see if we have anything that might be a better fit.

the balance itself is in good shape, what about the top and bottom plastic pieces as well as the rivets to put them on.

If you wish to just replace the attachments, please start by watching our video tutorial guide on the best way to go about doing this:
Now, for your own balance, please take a look at our 19-005 top attachment, and either our 15-004 or 15-005 bottom shoe. It looks like you might have the 15-004 shoe, but like I said before please double check its measurements to make certain. While there is only a fraction of an inch difference between these two shoes, it is in fact vital to the operation of your balance, and we highly recommend that customers get as exact a measurement as they can.
For the rivets, please take a look at our 62-005.
