Mark, thank you for the additional pictures.
It looks like you have either our Series 380 or Series 385 channel balance.
The difference between the two series is in the bottom shoe attachment.
Comparison of 15-004 and 15-005
The series 385 balance has the 15-004 shoe which is slightly larger from front to back, as illustrated.
If you measure your shoe as it is pictured here, you should be able to determine which one you need.
Whichever series balance you choose, the stamp option will be 3350 based on your pictures.

The bottom shoe is 15-005.

Take a look at the link below for the replacement balances you need.
Based on the pictures you submitted, you will choose the 3350 stamp option, which is for windows weighing between 28 and 32 pounds.
