The plastic clips that hold the channel in place have either come loose or have cracked.

It sounds like you're only looking for the shoe. If that's the case, see the 15-236 with a closed cam. This looks like a good match but review the specifications and let me know if you note any discrepancies.

We can help with the balances if you need them, they'll just need to be a custom order since yours appear to use a hook for installation. Let me know!
